HOME AND COMMUNITY BASED SERVICES POLICY

1. Obtaining Individual Choice

HWHI Lead staff and DSP's will talk to all individuals and discuss their preferences in activities and any goals they have. These preferences will be shared with staff to help brainstorm potential community events and activities in which the individual may wish to participate. Staff will talk to individuals at least weekly about their schedules and integrate any feedback. All individuals will be encouraged to pursue their own interests and individualization will be clear through their Individual Support Plan (ISP) and weekly schedules. 

 

Individuals can choose to decline services without any negative consequences imposed by Home Is Where the Heart Is.  If an individual cannot safely spend time alone, companion services will be used during that time period.
